Incident Learning Review Training
This course aims to improve the skills of Incident Learning Review (ILR) chairs and embed an educational approach throughout the review process. It focuses on reflection and improvement. This supports better performance, safety, and resilience across the rail network.
Course overview
This course has been developed to close critical gaps in how the rail industry learns from events that affect performance.
It directly responds to findings from the T1336 research project. This project found three persistent challenges:
- Learning practices that were inconsistent
- Limited dissemination of shared insights
- Missed opportunities to enhance safety and service recovery.
The course tackles these issues. This helps to strengthen industry-wide learning and improve operational resilience. It also equips IRL chairs to lead reviews that drive change and prevent repeat incidents. Delegates gain tools to improve resilience in operations, reduce delays, and strengthen safety outcomes.
This course gives delegates practical strategies and leadership tools so IRL chairs can foster a culture of accountability and continuous improvement.

Learning outcomes
- Lead IRLs with confidence and clarity
- Describe the three key steps of the learning process
- Identify and learn from factors that lead to incidents
- Apply the ‘Five Whys’ technique and use ten factors to explore the root causes of incidents
- Use ‘TED’ questions (tell me, explain to me, describe to me) to gain insights
- Develop SMART (specific, measurable, achievable, relevant, time-bound) actions to learn from findings
- Equip you to promote a learning culture throughout the ILR process.

His experience spans major industry projects in research, safety, and operational enhancement for companies, regulators, and industry bodies. He is a chartered psychologist and registered with the Health and Care Professions Council (HCPC) as an occupational psychologist.

Trevor is an experienced transport professional. His 25+ years in the industry span systems research, technology strategy, and innovation delivery. He has designed and delivered training in both technical and operational topics, working with a wide range of rail industry stakeholders. Trevor facilitates engaging, collaborative workshops. His training sessions convert complex information into actionable insights.
