Strategic Accident Prevention with Applied Human Factors

This paper discusses human error in relation to the aviation industry. It suggests that almost 100% of aviation incidents and accidents can be attributed to human errors within the system. This is because personnel have incentives, capabilities and the responsibility to maintain the safe and effective operation of the system.
