Agents and actions: Structuring human factors accounts of major accidents

This paper presents a notation methodology for representing accident data. Problems in current accident reporting techniques are identified as not making the impact of human factors upon an incident clear to the reader. This notation aims to remove many of the ambiguities, and highlights the human interaction with the machine/system.
