This Guidance Note is intended to promote the adoption of standard procedures for the reporting and investigation of station stopping incidents across the industry, along with a common definition of what is meant by these terms. In particular, it provides a suggested template pro-forma for the capture of all data needed to support such investigations. Though originally issued in February 2013, this document has not previously been published by RSSB (where is supersedes the previous version).
