Our monthly update on incident investigations around the world is available to download from our website.

In this month’s update, you can read about the investigations and updates published following four incidents, in the UK, Bulgaria, Slovak Republic and Australia.  Incidents include a collision between a train and a wooden sleeper in Suffolk (June 2015), a collusion at Plovdiv, Bulgaria (August 2015), a signal passed at danger at Bratislava-Lamač, Slovak Republic (October 2015) and a passenger train derailment in Sydney, Australia (January 2014).

The update is free to download from our site and includes a brief incident synopsis, along with the main causes and recommendations from each investigation, published last month (November).
