A signal passed at danger – or SPAD – occurs when a train goes past a red signal without authority.
All category Signals Passed at Danger (SPADs) are recorded within the Safety Management Information System (SMIS) and had historically been assigned a severity ranking and a hazard ranking index based on what actually occurred as a result of the SPAD.

The SPAD Risk Ranking Tool gives a measure of the level of risk associated with each SPAD that occurs, and as such enables the industry performance to be measured in terms of a percentage change year on year. Additionally, the SPAD Risk Ranking Tool can be used more broadly to inform the SPAD investigation process.
