Annual Health and Safety Report 2019/20
The Covid-19 pandemic has brought about profound change, with reduced services and trains carrying far fewer passengers. What hasn't changed is our commitment to keeping everyone who uses our railway and everyone who works on it healthy and safe.
Rail remains one of safest forms of transport, and the industry has a track record to be proud of. Britain’s railway is the safest of the top 10 biggest railway systems in the EU, and there has not been a train accident involving a fatality to passengers or the workforce on-board a train for over 13 years. However, there are still areas of risk that need focus from industry to effect improvements.

The Annual Health and Safety Report 2019/20 combines the latest information on health and safety performance, operational learning, and risk reduction initiatives. It takes into account reports to the Confidential Incident Reporting & Analysis Service (CIRAS). It also gives an overview of progress in implementing the strategy in Leading Health and Safety on Britain’s Railway (LHSBR). We identify where things have gone well and some areas of concern.
This year the AHSR is supported by 14 topic-specific reports:
- Rail Safety in Context 2019/20 provides an overview of rail safety in 2019/20.
- Improving our Capability 2019/20 provides an update on industry activity to improve risk management capability.
- Twelve reports structured around the LHSBR priority risk areas:
- Workforce health and wellbeing

- Fatigue risk management
- Workforce safety
- Infrastructure asset integrity
- Work-related violence and assaults

Periodic safety performance reports
These reports provide information on fatalities, reportable train accidents and accident precursors for the period in question. Graphic representations of trends are also included. We use the latest information available when producing safety updates, based on data available at the time of running the report. This is subject to change if further information becomes known; therefore, small differences may occur between updates.

LHSBR quarterly progress reports
The LHSBR quarterly progress reports provide an update on what the industry is doing to meet the strategy’s aims and objectives and includes safety performance data. These reports are published in September, December and March of every financial year, the final update in June is replaced by the Annual Health and Safety Report.
Signals Passed at Danger and Train Protection and Warning System Report
A report summarising signals passed at danger (SPAD) performance and Train Protection and Warning System (TPWS) activity can be accessed by RSSB members here.
Precursor Indicator Model
The Precursor Indicator Model (PIM) measures the underlying risk from train accidents. This is updated periodically and can be accessed by RSSB members here.
Injury categorisations and weightings
The rail industry measures safety performance and risk using the fatalities and weighted injuries (FWI) measure. In 2021 changes were made to the injury categorisations and weightings that the rail industry uses to calculate FWI for safety performance reporting and risk modelling.
