SPAD Investigation Report Template

SPAD investigation reporting provides an opportunity to learn from SPAD incidents and prevent them from reoccurring. However, the quality of reporting varies across the industry. The SPAD Risk Sub-Group (SPAD RSG) have developed a template to promote consistency and good practice when reporting on SPAD investigations.
