Identifying the Underlying Causes of SPADs

We can understand SPAD incidents better when we include underlying and contributory factors. By considering what the driver has or has not done, together with a wider set of equipment, organisational, design, and environmental factors, we can make effective recommendations to reduce the risk of future SPADs.
