When Software Goes Wrong
While it's relatively new to us, software is now everywhere: on trains and infrastructure, and often interacting. This series addresses recommendations from RAIB in its report on the Cambrian Line Incident: particularly that we must get better at reporting and recording when software goes wrong. Our excellent safety record is based on knowing what went wrong and planning for it not to happen again. Only if you report software failures will we be able to prevent future software-based incidents.
