Highlights from Period 12 (1 February 2026 to 28 February 2026)

Workshop on reducing the impact of fatalities on the network

 

RSI attended a workshop on reducing the impact of fatalities on the network. The workshop had representatives from Network Rail, British Transport Police, and train operating companies. One topic covered was our knock-on risk research, which received excellent feedback from attendees. RSSB took actions to operationalise findings from the knock-on risk analysis and to support an industry process review. These will help tackle trespass-related delays and manage stranded trains.

 

Safe Insights data quality

RSI provided 33 user organisations of Safe Insights with detailed data quality scorecards. Of these, 17 achieved Gold standard and 12 achieved Silver, two a bronze and two on requires improvement. The scorecards summarise outcomes from continuous data quality monitoring, training and engagement, and one-to-one health check meetings with each operator. One area where we have seen substantial improvement is the completion of SPAD Cause information with 95% of SPADs from 1 April 2021 – 31 October 2025 having a cause (compared with 51% at the end of 2024).

Safe Insights event migration

In a collaboration between RSI and Digital, we have finished migrating SMIS events to Safe Insights. This means that our members can now edit historical records in Safe Insights and automate reporting for all types of safety events.

Publication of T1316

We have published our research on assessing the case for rolling stock and infrastructure design features that can provide guidance to trains when derailed (T1316) and developed an industry-agreed strategy, which will close Recommendation 12 from the RAIB’s Carmont investigation. Network Rail are using the risk model developed by RSI to target guard rail fitment to improve safety and avoid unnecessary cost. The project also recommended that Network Rail evaluate and trial twin-block sleepers, and RSSB update Key Train Requirements for new rolling stock design.

London St Pancras High Speed meeting

We presented at the London St Pancras High Speed (LSPH) quarterly assurance board meeting on 25 February. We presented an overview of some of the key safety themes from the past year in the railway industry, including vegetation management, passenger behaviour, weather and climate resilience, and overspeeding, and we provided an update on high speed rail incidents that occurred overseas in the last year and a discussion on the Network Rail High Speed Safety Risk Model project. The presentation was well received and sparked wider discussion on the risks and challenges identified across the rail industry and how they compare with Network Rail High Speed's.
