Synopsis
This form is used by the signaller, COSS and an IWA to record the details of a line blockage as set out in modules T2 and T12. There are no examples of this form shown in modules T2 and T12 at present. An example of the form can be found in GERT8001, Changes to Specified Operations Publications as amendment number 12/06s and 13/06f.
