Signals Passed at Danger (SPADs)

Synopsis

This document sets out the requirements for managing signal passed at danger (SPAD) incidents in a way that ensures that risk following the SPAD is controlled. The document also sets out requirements to ensure that each SPAD is investigated in a consistent and structured way in order to prevent, or reduce the probability of, recurrence, without apportioning blame or liability.
