Signals Passed at Danger (SPADs)

Synopsis

This document mandates the requirements for managing signal passed at danger (SPAD) events in a way to mitigate risk following a SPAD. It also mandates the requirements to ensure that each SPAD is investigated in a consistent and structured way so as to establish the full facts and causes, in order to prevent, or reduce the probability of recurrence.
