This document sets out requirements for the investigation of accidents and incidents involving more than one duty holder so that system improvements necessary to prevent or reduce the likelihood of recurrence, or mitigate the consequences, are identified and implemented.

RIS-3119-TOM reproduces GORT3119 in its entirety, unchanged in Annex A. This RIS can be adopted by IMs and RUs to help discharge their responsibilities for the investigation of accidents and incidents involving more than one duty holder on the GB mainline railway, and it will provide a basis for co-operation between IMs and RUs when planning responses to incidents.
