Form RT3119-D Iss 4

Infrastructure managers and railway undertakings shall use the RT3119 designated Provisional SPAD Data Collection forms when investigating signals passed at danger incidents. Clause 5.10 was incorrectly referenced in Issue 3, as the clause does not exist. This should instead refer to clause 5.8.

In Issue 4, Clause 5.9 is amended to read: "If 'Yes' to 5.8, what was the nature of the unusual circumstances? Tick one."
