RIS-8047-TOM Iss 2.1

The document sets out requirements for entering information into the Safety Management Intelligence System (SMIS). SMIS has been replaced by the Safe Insights reporting tool. 

All references to SMIS, except for historical mentions in the introduction, have been removed and replaced with references to Safe Insights. Some of the existing capabilities of the SMIS system had been switched off as they were not utilised. References to recommendation tracking as part of this have been removed.
