This form is used by signallers to record the details when personnel ask for a line to be blocked for their own protection when going on or near the line.

Two changes have been made to the form to reflect operational feedback received since it was first implemented in December 2024. The ‘Location of train’ field has been changed to read ‘Location of requestor’. If, for example, platform staff are asking for trains to be stopped to retrieve a dropped item from the track, there is no train directly involved, and ‘location of train’ is not relevant to the request.

The number of lines in the ‘Protection details’ section has been extended from three to six lines as general signalling regulation 13.1 requires the signaller to block all lines at a given location if the person asking for trains to be stopped is not sure which lines they need.
