Reporting of adverse events

Synopsis

This technical note will help industry to understand the reporting systems for safety related issues provided by the Rail Safety and Standards Board and when to use them.

Summary Of Changes

This technical note has been produced to support compliance with legislation such as the Railways and Other Guided Transport Systems Regulations 2006 (as amended). The Rail Safety and Standards Board provides several systems to help industry discharge their responsibilities for assessing, sharing, and reporting safety-related information.

This technical note complements a number of standards related to reporting adverse events.

It is applicable to:

  • infrastructure managers;
  • railway undertakings;
  • rolling* stock owners and leasing companies;
  • vehicle* maintainers
  • vehicle* component and system suppliers and integrators* and control command signalling subsystems;
  • entities in charge of maintenance;
  • safety professionals using the Safety Management Intelligence System; and
  • statutory bodies.
