Our monthly update on incident investigations around the world is available to download from our website.

In January’s update, you can read about the investigations published following five incidents, two in Australia, one in Germany and two in the UK.  The findings cover issues such as signals passed at danger, freight derailments, and maintenance work.

The update, prepared by our Operational Feedback Specialist, Greg Morse, is free to download from our site and includes a brief incident synopsis, along with the main causes and recommendations from each investigation, published last month (January).
